Quoting:We quote medical coverage from all the major medical insurance carriers. For Self-Employed Business owners in New York, we can write through either of the associations we participate with; FNA - First National Administrators or NYSBG - New York State Business Group / Conference Associates. For Group Businesses located in New York State that are between 2 - 50 employees, we quote in house using Health Search, Group Quote, PGP Quote and Guardian Proposal Partner 2. For Group Businesses located outside the New York State area or for 50+ employees, the medical carriers require a census (consisting of gender, dates of birth, home zip codes and family status for all employees). We send this census information out for a quote to our representative at Professional Group Plans; a general agency who handles large group and out of state quotes for us. Additions:We encourage the benefits administrator to fax us any additions to your plan. Additions qualify as the following: a new employee, a rehired employee, a spouse (due to marriage or loss of coverage from their company), the birth of a newborn or the adoption of a child. You have within 30 days to enroll. Please make sure that all full names, dates of birth, social security numbers, addresses, etc. are correct and legible. We will send them to the carrier and check on it to affirm its completion. Terminations:We encourage the benefits administrator to fax us any terminations on your plan. Terminations qualify as the following: an employee who resigned, an employee who has been terminated, a spouse or child of an employee who passed away, a spouse or child of an employee who is obtaining other coverage or a child who is over the dependent status limit. You have within 30 days to terminate. Please make sure that all full names, dates of birth, social security numbers, addresses, etc. are correct and legible. We will send them to the carrier and check on it to affirm its completion. Physician Directories:We are able to search for physicians through Health Search’s Pro Access and Quote’s Doctor Directory. Sometimes we may require you to have readily available to us the following: Physician’s Full Name, Specialty, Office Location(s), and Phone Number, so we may narrow down our search or contact him/her for carrier verification. Maintenance Medication:If you or your employees are taking important maintenance medication that is vital in your decision for group health coverage, please let us know. Simply provide us with a list of prescriptions that you and your employees are currently taking and will find a plan that accommodates everyone’s needs and inform you on the tier structure and cost for each prescription for each of the quotes that we provide. Claims:We encourage the benefits administrator to fax us any claims problems that may accur. We will then call the carrier to ensure that your claims are paid. Billing:Forest Hills Group Services does not receive your bills each month. If you have a billing issue, kindly fax us a copy of the bill(s) in question attaching a fax cover sheet and describing what the problem is and what kind of modification should be made. We will contact you with a result shortly. Most health insurance companies do not mail an adjusted bill - the adjustment will show up on your next billing statement. Benefit Questions:We are here to answer all questions about benefits. Some questions require us to contact our sales representatives, as they have first hand knowledge of each carrier, so your patience is appreciated. This enables you to make one phone call to us, without the hassle of calling five departments and receiving no response. We will call you with a proper answer in a timely manner. Research:Our Group Services Department will research any questions you might have about state mandates (ie: infertility, diabetes, etc.), state tax documentation for group submission and questions about extended benefits within your plan such as durable medical equipment, skilled nursing facility and mental health. We will inform you about the requested information via phone, fax, mail or email. Renewals:We contact all our clients upon renewal with each carrier via mail. This mailer includes the Renewal Letter from the carrier, a Forest Hills Group Services letter and plan options. If you would like to renew your policy as is with no charges, simply fax us a copy of the renewal letter stating ‘Renew As Is’. If you would like to make changes to the policy, please call us before the time frame located on our letter and we will modify your plan accordingly. |
